Can anyone who's spent any time at all considering the health care "reform" possibly be surprised by
this?

This is a good thing, John. We need lots of pain, and we need it before November. Actually, I'm surprised that the Democrats were so sloppy. Usually, in pernicious legislation like this, the pols ease us into it, thereby locking us in for the long haul.
Our only chance of ever getting rid of this monstrosity depends on the brief window from now until November. We need a debacle, and we need it this summer. Employer-provided health insurance premiums going way up is as close as we''re likely to get to the needed disaster.
